Govt. conspiracy. They were warned and didn't act, creating a theatre of fear was most ingenious. If they had carried on into Iraq during the gulf war and removed Hussain from power like Schwartzkopf wanted to do and not just stopped at the border, the whole thing would have been far less likely to happen. If we ever stop fighting with the Muslims over this nothing will ever repair the damage to race relations.

I thought they drove him out of Kuwait but didn't move on into Iraq, is that not how it happened?

No, there was a major ground offensive by 2 US corps, a UK Div plus French and Canadian forces that destroyed the Iraqi army inside Iraq, stopping just short of Baghdad, where Bush Snr called a ceasefire. The Turkish army also deployed along Iraq's northern border, which meant Saddam had to split his forces.
There was no mandate to destroy the regime, the coalition would have fragmented, the US didn't want to occupy (it's an expensive business) and in those heady 'end of history' and peace dividend days there may have been some wishful thinking that the Iraqis would overthrow Hussein themselves.
Iraq was never a haven for al-Qaeda or the Taleban, they were always in post Soviet Afghanistan.

I'll leave the conspiracy theory topic for fans of the X-files.
I think conspiracy theorists in general tend to have a bit too much time on their hands and a not too solid grasp on reality.
