You said you were pedaling hard and the chain jammed - sounds like it could have been the freewheel jamming.............? Like Han said get it checked out but make sure they check the freewheel as well.

Oh dear, you have been in the wars.....hope it heals quickly....have you though about padding?!! ;-)
