I would think a trip to a good physio may offer you some answers/solutions.

I have a similar problem Fat Lad (see my previous posting) and have since been to see a physio. Her diagnosis is that I have one leg slightly shorter than the other and - even though I have been running for over 30 years now - the consequences become more pronounced as you get older (or heavier?) Her advice is a variety of calf and hamstring stretching and strengthening activities that take up about an hour a day just now. I have tried orthotics in the past but -especially on the fells where your foot rotates more than on the road - have found that they cause more problems than they solve. Go and see a physio though, because our injuries are all different: mine is excellent (NHS) and spent 50 minutes on the the first session with me diagnosing and advising. Good luck
